8. In Section F: Other proposed changes to your current position, describe any additional changes you wish to request.
9. Complete Section G: Declarations by reading and acknowledging the privacy statement followed by signatures from you, your principal supervisor, and employer sponsor where applicable.
10. Finally, check the Checklist in Section H to ensure you have attached all required documents and submitted the completed form to AHPRA.
11. Once completed, save your changes, and if necessary, download or print the form for your records before submitting the application.
