6. You will need to give your consent and declarations in Section E, ensuring you understand the terms related to your personal information and funding requirements. Sign and date the application as required.
7. If applicable, Section F must be completed by a disability advisor to confirm the necessity of the services or equipment requested.
8. Finally, ensure all sections are accurately filled out, required documents are attached, and submit your completed application either to your financial aid office or the designated ministry address, as applicable.
9. Once the form is submitted, keep copies, and track any follow-up requirements. After receiving approval, manage the receipts for approved purchases to fulfill funding obligations.
