10. Check the box that indicates your readiness for the program and provide your student signature and date.
11. Have your parent or guardian sign the application and note the date.
12. Attach the required documents: a current resume, an unofficial copy of your high school transcript, and two letters of recommendation. Ensure all files are ready for submission.
13. Finally, review your application for accuracy before saving changes. You may choose to download, print, or share the form as needed.
