9. Choose three words that best describe the student and explain each choice.
10. List any special academic programs, clubs, or competitions the student has participated in over the past three years, providing details about involvement and leadership.
11. Include a statement from the parent or legal guardian explaining how the PROMISE program can enhance the student's education.
12. Obtain signatures from both the student and the parent or legal guardian to certify the accuracy of the information provided.
13. Review the completed application for accuracy and save the changes. Then, you can download, print, or share the application as needed.
