Follow the steps to successfully fill out the form.

2. Begin by filling out the child information as currently displayed on the STAC Online (EFRT) System. This includes the child's last name, STAC ID, middle initial, first name, gender identity, date of birth, and race-ethnicity.
3. In the section labeled 'Change Child’s First Name to', enter the new first name if applicable. Similarly, fill out the fields for middle initial, last name, date of birth, gender identity, and race-ethnicity as needed.
4. Provide the reason for the change by selecting one of the options, or write 'Other' to specify. If the child was adopted, fill in the date of adoption.
5. Ensure to include the name of the person processing this information and their title in the designated fields. This individual may be the district superintendent, municipality representative, or director of pupil personnel services.
6. Don't forget to enter the date and provide a contact email address and phone number for follow-up purposes.
7. Once all fields are completed, review the information for accuracy and completeness. Save changes to the document, and proceed to scan and upload the completed form to the SED File Transfer Manager (FTM) ‘inbasket’.
8. Email the designated contact at OMSSTAC@nysed.gov with the SED FTM location and filename to notify them of the submission, ensuring you do not attach the completed form to emails.
