5. If you select Part Two, Section B, have your supervisor complete Part Three. They must record the hours they supervised you, sign and date the application, and include their license number. Each supervisor must fill out a separate form if there is more than one.
6. For all applicants, ensure compliance with the child abuse recognition and reporting training requirement. Complete three hours of approved training, and the provider will electronically submit your details to the board after completion.
