- Indicate if you want your address printed on your licence by selecting 'Yes' or 'No'. Providing your contact phone numbers and email is optional.
- Decide if you require a new photo and signature for your driver licence or a new photo on your driver ID card.
- Specify the reason for your replacement request by selecting from options such as name change, lost, stolen, or change of appearance.
- Complete the declaration section. Confirm you are surrendering your current card, or explain why you are not.
- Review all information entered on the form to ensure accuracy, then you may choose to save changes, download, print, or share the completed form.
