9. If applicable, answer the questions about previous funding applications within the fiscal year and disclose any additional funding sources you may have received.
10. Sign and date the application at the bottom to validate your submission.
11. Once you have completed all sections, mail the finished application form to the address provided in the instructions. Make sure to send it to the Continuing Education Fund—faxed applications cannot be accepted.
