You may apply for a replacement Persons with Disability Parking Placard by completing Form MV-145A, “Persons with Disability Parking Placard Application.” Your previous placard number will be listed as the previous placard on PennDOT's computer system.

Identifying license plates or placards can be issued to a person by submitting a completed DR 2219 Persons with Disabilities Parking Privileges Application to the County Motor Vehicle Office. The application must be signed by the applicant or a representative.

The nearest place you can go to get a handicap parking permit in-person will be either your local Department of Motor Vehicles office or your local tax collector's office.

(3) No person shall make, issue, possess, or knowingly use any imitation, counterfeit, or transferable placard or license plate for a person with a disability.
