Follow the steps to complete the VAT declaration form.

1. Click ‘Get Form’ button to acquire the VAT Declaration form and open it in the editor.
2. Enter your account reference number. This number can be found on your invoice.
3. For each Meter Serial Number that qualifies for VAT reduction, complete a separate form.
4. Fill in your business, company, or organisation name in the designated field.
5. If applicable, provide your VAT number or charity registration number.
6. Input the premises address where the VAT declaration applies.
7. Indicate the percentage of supply you claim qualifying use on, ensuring it's an exact percentage only if there is a shared supply.
8. Complete the premises postcode.
9. Select the appropriate description of your claim by ticking the relevant box: Domestic use, Charitable non-business, or Other. If 'Other' is selected, provide a reason in the space provided.
10. If your registered office address differs from the premises address, fill in the registered office address.
11. Specify the date from which you wish to apply the VAT reduction. Note that this can be back-dated for up to four years.
12. Complete the registered office postcode.
13. Provide the full name and job title of the authorised signatory.
14. Affix the authorised signatory's signature and date the declaration.
15. Review the declaration to ensure all information is correct before sending it back to contactus@opusenergy.com.
