11. In the Parent/Guardian Consent section, confirm consent for student participation during COVID-19 by entering the parent’s signature and the date signed.
12. For the Consent for Release of Confidential Information, the student and parent will also need to provide their signatures and dates.
13. Finally, ensure all fields are accurately completed and submit the form as directed by your school’s Career Coordinator.
14. Once the form is submitted, you may save changes, download, print, or share the completed form as needed.
