16. Inform how long you have continuously resided abroad.
17. Clarify your citizenship status — whether by birth, descent, naturalization, or registration.
18. Disclose if you have ever possessed any other nationality or travel document, along with relevant details.
19. Indicate if you have ever been refused an Indian passport or if your passport has ever been revoked.
20. Provide the names and addresses of two relatives or friends located in K.S.A or India.
21. If you are in government service or a public undertaking, provide details and include a 'No objection certificate' from your employer.
22. Detail any pending criminal proceedings against you and whether you have ever been repatriated at the government’s expense.
23. List the number of any lost or damaged passports including relevant issuance details, and briefly explain the circumstances surrounding the loss or damage.
24. Complete the final sections including registration with the Indian Mission/Post and next of kin details for emergencies.
25. Affirm your declaration, ensuring you sign within the specified box. Review all information for accuracy before submission.
