- In Part C - Performance Monitoring and Progress Review, document at least one progress review during the appraisal period, completing Section I with identifying information, and collaboratively filling Section II with narratives from both the Rating Official and the employee.
- Section III requires both the Rating Official and employee to sign after the progress review is completed.
- Move on to Part D - Performance Rating. Complete Section I with identifying information. In Section II, if desired, the employee can provide a self-assessment narrative.
- In Section III, the Rating Official evaluates each critical element, documenting ratings and supporting narratives as necessary.
- In Section IV, the Rating Official assigns a summary rating based on performance and forwards it for potential performance recognition in Section V.
- Obtain all required signatures in Section VI, ensuring that signatures acknowledge the contents of the document without necessarily indicating agreement.
- Utilize Section VII for any additional documentation if needed.
- Once the form is fully completed and signed, save your changes, and proceed to either download or print the form for submission.
