Appraisal period is 90 days. At the end of the appraisal period, employee has 7 days to provide a self assessment narrative (optional), which will be considered by the Rating Official when preparing the summary rating. Within 20 days of the end of the appraisal period, the Rating Official shall compare employee performance against the established critical elements and performance standards; consider the employee's self assessment; rate each critical element; and assign a recommended summary rat.

  11. In Section IV, the Rating Official assigns a summary rating based on performance and forwards it for potential performance recognition in Section V.
  12. Obtain all required signatures in Section VI, ensuring that signatures acknowledge the contents of the document without necessarily indicating agreement.
  13. Utilize Section VII for any additional documentation if needed.
  14. Once the form is fully completed and signed, save your changes, and proceed to either download or print the form for submission.
