  7. Fill in your complete mailing address, ensuring to include any apartment or P.O. Box details.
  8. Include your 9-digit zip code, which is required for bulk mail. You may find this information on various mail pieces or via postal service online tools.
  9. Add your home and cell phone numbers, including the area code, followed by your email address.
  10. Leave the 'FOR OFFICE USE ONLY' section blank as instructed.
  11. Proceed to Page 2 and complete the section regarding your direct-line Utah pioneer ancestor(s). List at least one ancestor, providing their name, birth date, birth place, and relationship to you.
  12. Detail the year your pioneer ancestor arrived in Utah and the company they traveled with during their journey.
  13. Record where your pioneer ancestor first settled, using the present-day state.
  14. Fill in the death date and place for your pioneer ancestor if applicable.
  15. Move to Page 3 and fill in your pedigree chart, tracing your lineage from your parents to grandparents, back to the direct-line ancestor mentioned previously.
  16. In the space provided, write a brief history of your chosen direct-line Utah pioneer ancestor, avoiding any attachments or exceeding the space limit.
  17. On Page 4, input your full name if it's not already there, and list your children’s full names along with their birth dates.
  18. If you are filling out the form online, print a copy of your completed application. Remember that the application may not save automatically.
  19. Read the pledge and sign where indicated as this document requires a legal signature, which must be witnessed by someone over 18 years of age.
  20. For members-at-large, obtain two witness signatures, and send your application along with a payment of $20.00 directly to the designated address.
  21. If applying through a camp, submit your application to the camp registrar along with the payment. The registrar will assist you with finalizing details.

Membership in the organization is open to any woman who is "over the age of eighteen years, of good character, and a lineal or legally adopted descendant of an ancestor who came to Utah before the completion of the railroad, May 10, 1869."
