6. If the trip is organized by a student organization, indicate if crowdfunding or fundraising will be utilized to cover trip expenses.
7. List all proposed trip sites, including cities, towns, and countries, ensuring to be as specific as possible.
8. Enter the proposed dates abroad in the provided fields.
9. State the number of anticipated participants, excluding staff and faculty.
10. Specify the number of accompanying CUNY staff or faculty expected to join the group.
11. Provide the name and title of the field director, along with the name(s) and title(s) of other staff or faculty accompanying the group.
12. Indicate whether any trip locations are under a Department of State Travel Warning or a Level 3 CDC Travel Warning and attach a waiver petition if necessary.
13. Attach a printout of current CDC recommendations for travelers to the trip destination and indicate necessary vaccinations and medications.
14. Describe the field director's qualifications related to the destination.
15. Provide information on accommodations planned for the trip.
16. Describe transportation methods for local travel, noting that CUNY faculty, staff, and students cannot drive motor vehicles for trip purposes.
17. Confirm whether participants will be covered by the CUNY-sponsored CISI travel insurance; if not, attach details of an alternate plan.
18. Note if high-risk activities are included in the itinerary and provide a justification and risk mitigation measures.
19. Ensure a pre-departure orientation will occur, documenting participation for two years.
20. The Campus Director must certify and provide their contact information, followed by the Responsible Executive Officer's approval.
21. Upon completion, submit the form electronically in PDF format, preferably along with any attachments, to the Campus Director and the email global@cuny.edu.
