- Attach a printout of current CDC recommendations for travelers to the trip destination and indicate necessary vaccinations and medications.
- Describe the field director's qualifications related to the destination.
- Provide information on accommodations planned for the trip.
- Describe transportation methods for local travel, noting that CUNY faculty, staff, and students cannot drive motor vehicles for trip purposes.
- Confirm whether participants will be covered by the CUNY-sponsored CISI travel insurance; if not, attach details of an alternate plan.
- Note if high-risk activities are included in the itinerary and provide a justification and risk mitigation measures.
- Ensure a pre-departure orientation will occur, documenting participation for two years.
- The Campus Director must certify and provide their contact information, followed by the Responsible Executive Officer's approval.
- Upon completion, submit the form electronically in PDF format, preferably along with any attachments, to the Campus Director and the email global@cuny.edu.
