2. Begin by filling in the alumni's particulars section with the necessary information: Name, Course, Post, and Name of Company.
3. Answer the first survey question by indicating whether the individual is still employed with the company by selecting 'Yes' or 'No'.
4. For the relevance of the UTAR curriculum, respond to each statement by ticking the box that corresponds with your assessment on a scale of 5 (Strongly agree) to 1 (Strongly disagree). This repeated for multiple statements regarding different aspects of their education.
5. Evaluate the proficiency in English and any other commonly used language for work by selecting a score from 5 (Excellent) to 1 (Very Poor).
6. Provide an assessment of the individual's attitude towards job functions and responsibilities by scoring appropriately from 5 to 1.
