If you already have a Michigan UIA Employer Account Number, look this up online or find it on correspondence received from the Michigan Unemployment Insurance Agency. If you are unsure, contact the agency at 855-484-2636 or OEO@michigan.gov.

The agency began the waiver process for overpayments it caused by using unauthorized criteria in June 2021, asking nearly 650,000 PUA claimants to requalify and/or recertify for benefits, using the specific federal criteria only.

You have the right to appeal an overpayment determination. You must submit your appeal within 30 days of the mailed date on the Notice of Overpayment or the Notice of Denial of Benefits and Overpayment. You can download the Appeal Form (DE 1000M) (PDF) or use the copy included with the overpayment notice.

The UIA will send you a determination that says you have committed intentional misrepresentation if it thinks you've committed fraud. If you get a determination that says you have intentionally misrepresented information to the UIA, it may include information about the money you have to repay.

You will need to contact U.S. Bank to order a replacement card through their automated system. Call 866-335-1890.

You can submit your protest through your MIWAM account. After accessing your MIWAM account: Click on the claim ID of the involved benefit year to view the details. Next, click on "Determination Status", and then click on "file a protest" or "file appeal" for the issue you wish to protest.
