Before you register, contact the GP surgery to confirm that it covers the address where you are living or staying temporarily. Ask to register at the GP reception. No, you don't have to tell your current GP that you want to change. You don't have to tell the new GP surgery why you want to change, either. You'll need to fill in a registration form. A request will then be made to your current GP for your medical records to be transferred to the new GP surgery.

The NHS has always involved a mixture of public and private provision. For example, most GPs are not public employees but rather independent contractors to the NHS. This was part of the settlement struck with GPs when the NHS was established in 1948.

If you want a copy of the information in your records there may be a charge per copy up to a maximum of £50. If your records have not been made or added to within the last 40 days prior to your application, you will be charged a fee of £10 plus a charge per copy and postage, if applicable.

Registering with a GP When you have found a practice you like, you'll have to formally register with it as an NHS patient by submitting a registration form to them. The GMS1 registration form (PDF, 156kb) is available at the practice, or you can download it from GOV.UK.

To change your GP, you should contact the surgery you want to change to, and ask if they will accept you as a patient. If they agree to take you on, you will need to give them your medical card or fill in a form. When you have registered with the new surgery, your medical records will be sent to them.

It's quick and easy to register with a GP. Call Primary Care Support England on 0333 014 2884 (option 1) for advice, or pop into a practice near your home and ask about becoming a patient. You can also visit www.nhs.uk for more information. It's usually only possible to register with a GP near where you live.
