4. In Section 3, authorize the Special Needs Assistance for Seniors to contact the individual you specify. Input their name, relationship to you, and phone number for communication.
5. Section 4 requires signatures. Both the applicant and the spouse/partner (if applicable) must sign the form. Ensure you include the date of signing, reflecting your agreement to provide necessary information and account for the usage of any assistance received.
6. Review all entered information for accuracy. Once complete, you can save changes, download, print, or share the form as needed.
