8. Answer whether the participant is allowed to walk home, including the earliest time they may leave and the program they can walk from.
9. In the 'About Your Child' section, provide details about any medical conditions, allergies, behavioral concerns, and any special information that will assist staff in ensuring a safe environment.
10. Complete the prescription medication section thoroughly for any medications your child is prescribed, including name, dosage, and time taken.
11. Lastly, read and acknowledge the terms by typing your name as the legal parent or guardian to provide authorization.
12. Once all fields are filled accurately, save changes, and choose to download, print, or share the completed form as needed.
