8. Select whether a substitute will be needed for your absence. If yes, indicate which fund the substitute will be paid from and have your supervisor initial in agreement if necessary.
9. Indicate if there will be a cost associated with the travel. If yes, complete the 'Estimated Cost' section by providing amounts for hotel, mileage, airfare, registration, meals, taxi/shuttle, and any other expenses.
10. Sum the estimated costs in the 'TOTAL' section.
11. Finalize the form by obtaining the necessary approvals from your immediate supervisor, program director (if grant funded), director, chief academic officer, chief operations officer, or superintendent as required.
12. Sign and date the form to confirm all information is accurate. A copy of this form must be submitted to Payroll once all signatures have been obtained.
