- Indicate whether the volleyball net needs to be removed by checking 'Yes' or 'No'.

- Specify if alcohol will be served by selecting 'Yes' or 'No'. If 'Yes', you must complete the Request to Serve Alcohol form.
- Determine if food will be served. If so, indicate the source of the food by selecting 'Home/Campus Prepared' or 'Purchased Prepared for Resale'.
- Complete the sponsor information section, including the event contact person's details and the sponsor's university department or organization. Ensure that the sponsor contact person is a senior staff member or faculty member.
- Check the attachments that apply to your request, including the USC Events Permit Application Confirmation and any configuration maps if applicable.
- Review the general guidelines section to ensure compliance with all requirements. Obtain necessary approvals and sign the form as needed.
- Once the form is complete, submit it via fax to (323) 442-2434 or email it to hscquad@usc.edu.
