A Form 20 (Leave Authorization), also known as Extended Leave (EL), is a form under the British Columbia Mental Health Act that is completed and allows an involuntary patient to leave the hospital and live in the community.

A Form 4 (Certificate of Renewal) under the Ontario Mental Health Act is a form issued when a patient continues to meet criteria for an involuntary admission after a Form 3 expires.

When a patient is discharged from involuntary status, a Form 19 must be completed and signed by the director or designate. Once written notification of discharge has been provided, any Medical Certificate (i.e. - Form 4) or warrant providing authority for hospitalization is cancelled.

What is extended leave? Extended leave means that the person continues to be certified under the Mental Health Act after discharge from a designated facility, such as a hospital or other mental health care location.

A Form 5 (Consent for Treatment), is a form under the British Columbia Mental Health Act that details the consent process for mental health treatment if a patient is involuntarily admitted on a Form 4 or Form 6.

A Form 10 allows police to apprehend and transport an individual to a designated facility and authorizes admission of the person for psychiatric assessment and treatment for up to 48 hours.

British Columbia's Mental Health Act allows us to admit and treat people with serious mental health issues who are a risk to themselves and/or others. People who are certified under the Mental Health Act can be treated involuntarily for a mental disorder.

A Form 47 (Order for Examination), can be issued by a physician when a patient is non-adherent to an agreed-upon treatment plan as laid out in the Community Treatment Order (Form 45). The patient is brought to a healthcare facility by law enforcement.
