Follow the steps to fill out your scholarship application accurately.

1. Click ‘Get Form’ button to obtain the form and open it in the editor.
2. Begin filling out your personal information. Provide your name, GPA, address, city, zip code, phone number, email, high school, ACT score, date of graduation, chosen college or university, and field of study.
3. Complete the work experience section. For each position listed, indicate the year, company name, phone number, address, and provide a brief job description. Up to four positions can be included.
4. Detail your awards and extracurricular activities. For each activity or award, specify the year, name of the award or activity, and include a description.
5. Write your short essays. You need to answer all three questions, each in 500 words or less: 1) What impact would receiving this scholarship have on achieving your dream career? 2) Why did you choose your field of study? 3) Where do you see yourself professionally in 10 years?
6. Ensure you gather and prepare three letters of recommendation, including at least one from a teacher or academic advisor. Remember to email these letters to khocklander@rggcardiology.com.
7. Review the entire form for accuracy and completeness. Make any necessary changes before you proceed.
8. After final checks, save your changes and download or print a copy of the filled-out form for your records.
9. Submit your completed application, essays, and letters of recommendation via Email before the March 15th deadline.
