p57 is a cyclin dependent kinase inhibitor protein, the product of the paternally imprinted, maternally expressed gene CDKN1C (p57KIP2) located on chromosome 11p15.5. p57 immunohistochemical stain can be used to separate a complete hydatidiform mole from partial hydatidiform mole and nonmolar gestations.

P57 protein is a marker distinguishing complete hydatidiform moles from partial moles.

On immunohistochemistry (IHC) with p57, complete mole (CM) shows absent staining whereas hydropic abortus (HA) and partial mole (PM) show positive staining.

Clinical Information p57 is expressed in the cytotrophoblasts, intermediate trophoblasts, and villous stromal cells in normal placenta. Loss of p57 expression is associated with complete hydatidaform moles, and can help distinguish them from partial hydatidaform moles and hydropic abortions.

p57KIP2 immunostaining has been widely described as an ancillary test to distinguish complete hydatidiform moles (CHM) and partial hydatidiform moles (PHM), but it cannot discern PHM from nonmolar gestations (NM) or identify androgenetic versus biparental CHM, which may be associated with increased recurrence risk.

p57 expression is highly correlated with genotyping and in practice can reliably be used to identify virtually all complete hydatidiform moles (CHM), but aberrant retained or lost p57 expression in rare CHMs and partial hydatidiform moles (PHM), as well as loss in some nonmolar abortuses, has been reported.

The p57KIP2 gene encoding for p57, a cell cycle inhibitor, is strongly paternally imprinted and expressed from the maternal allele.
