- In the Claim Information section, document the Date of Service, Diagnosis Code/Reason for incurring out-of-pocket expenses, Procedure Code, Charged Amount, and Paid Amount for each claim. Make sure to fill out all applicable claims, up to three in total.
- Sign the Acknowledgement section. Confirm that the information you have provided is accurate and true by signing as the Member or Authorized Representative. Enter the date of signing.
- Compile and attach Proof of Payment documents that show your name, such as a Doctor’s Receipt, Credit Card Receipt, or Cancelled Check (front and back).
- Ensure to keep a copy of the completed form and your receipts for your records.
- Submit the completed form and attached receipts to the appropriate address for your health plan, as listed in the guidelines.
