The investigator or the person responsible for collecting evidence must complete the labels of the sample container/bags and the chain of custody forms to enable tracking of the sample.

The term “Chain of Custody” form is commonly referred in acronym usage as a CCF or CoC, and references a document or paper trail showing the seizure, custody, control, transfer, analysis, and disposition of physical and electronic evidence of a human specimen test.

ASTM D4840-99(2018)e1: Standard Guide For Sample Chain-Of-Custody Procedures defines sample chain-of-custody as: “a process whereby a sample is maintained under physical possession or control during its entire life cycle, that is, from collection to disposal.”

The Relevance of the Chain of Custody Documentation Each sample container label must receive a unique identification code and other relevant information such as location, date and time of collection, the name, and signature of the person who collected the sample, and signature of the witness(es).
