5. In Part C, choose your payment method. Fill in cheque number and issuing bank if paying by cheque. If using e-invoice, provide the business unit code. For NETS or company GIRO, refer to the instructions provided accordingly.
6. Complete Part D by declaring that you meet the requirements for the course. Make sure to sign and date the form; if you are a company-sponsored applicant, authorized personnel must sign and stamp the form as well.
7. Finally, review the personal data protection act in Part E, sign, and date it. Make sure you understand how your data will be used.
8. Once the form is completely filled, save your changes. You may then download, print, or share the completed form as needed.
