6. In Part IV, assess the student's performance by checking the most appropriate box for each behaviour area listed. Categories include listening comprehension, oral expression, basic reading skills, and more.
7. Provide comments on any other relevant behaviours observed during the session and indicate whether the classroom teacher believes the behaviour observed is representative of the student’s usual behaviour in that classroom.
8. Once all sections are filled out, review the form for accuracy. You can then save your changes, download the completed form, print it, or share it as needed.
