Tests are performed on samples of saliva, serum, spinal fluid, and skin biopsies of hair follicles at the nape of the neck. If signs suggestive of rabies develop, the animal should be euthanized, its head removed, and the head shipped under refrigeration (not frozen) for examination of the brain by the Public Health Laboratory.

The only definitive way to determine if an animal had rabies is to examine the brain. This is impossible to do while alive. Blood and other ante-mortem (before death) tests are not reliable enough when a human life is on the line. This means cutting the head off and submitting it to a state lab for rabies testing.

Tests are performed on samples of saliva, serum, spinal fluid, and skin biopsies of hair follicles at the nape of the neck. Saliva can be tested by virus isolation or reverse transcription followed by polymerase chain reaction (RT-PCR). Serum and spinal fluid are tested for antibodies to rabies virus.

The sample should be sent immediately to the diagnostic laboratory closest to you. If the sample cannot be transported immediately, it should be stored in a fridge, +40C (for a few days) or freezer, -200C (for longer periods). Carcasses should be burnt, composted or buried if rabies is suspected.

If possible, submit the head of large animals and the entire body of bats. When submitting only the brain, submit the entire brain. Submit fresh, chilled tissues only; do not fix the brain in chemical preservatives. Refrigerate, do not freeze specimens for rabies testing prior to shipping.

Complete the Possible Human Rabies-Patient Information Form [PDF – 2 pages] detailing the clinical history of the patient and provide the name, phone number, and email of the physician who should be contacted with the test results.
