8. Complete the final sections I (Reassessment Status), J (Clinical Discharge Status), and K (Services Received), ensuring that all responses are recorded accurately.
9. After filling out the form, review all entries for completeness and accuracy. You can save changes, download, print, or share the completed document as necessary.
