- In Column B, enter the account number for each CollegeChoice 529 account for which you are claiming a contribution.
- In Column C, list the total contributions made during the year for higher education expenses corresponding to each account listed in Column B.
- In Column D, enter the total contributions made for K-12 education expenses for each account listed in Column B.
- After filling out lines 1 to 9, proceed to line 10 and add the amounts from Column C.
- Next, add the amounts from Column D on line 11.
- On line 12, sum the total from line 10 and line 11.
- Multiply the amount on line 12 by 20% and enter that result on line 13.
- For line 14, enter the lesser value among the amount on line 13, $1,000, or $500 if filing separately.
- Retrieve the amount from line 8 of your Form IT-40 or Form IT-40PNR and enter it on line 15.
- Finally, on line 16, input the lesser of line 14 or line 15, and include this amount under line 6 of Schedule 6 (for Form IT-40) or under line 6 of Schedule G (for Form IT-40PNR). Save your progress throughout the process.
