Get DoL Annual Refresher Training Record/Certificate

The MSHA 5000-23 Certificate of Training form is specified in Parts 46 and 48 as the Record of Training document required to “record and certify” that required training has successfully been completed. It's a form provided by MSHA for the purpose of standardizing training records for the mining industry.

How To Fill Out An MSHA 5000-23 Certificate of Training Enter the name of the person trained. Check the box of the type of training received. Check the boxes of the location and type of operation. Enter the date the training was completed. Check the box(es) of the subject(s) completed.

MSHA does not maintain copies of these records. Copies of training certificates must be maintained by the operator during employment and for 60 days after termination of employment.

The Certificate of Training Form 5000-23 provides a means for mine operators to record and certify Part 48 mandatory training received by miners.

MSHA's Approval and Certification Center (A&CC) approves and certifies certain mining products for use in underground coal and gassy underground metal and nonmetal mines. Technical experts evaluate and test equipment, instruments, and materials for compliance with Federal regulations.

At the completion of training, the instructor MUST give the miner a copy of the training certificate. When a miner leaves the operator's employ, the miner shall be entitled to a copy of his training certificates. The current MSHA Form 5000-23 has an expiration date of May 31, 2024 and the Approved OMB Number 1219-0009.

MSHA Part 46 Training Part 46 certification must be renewed every year with an 8 hour Annual Refresher training. There is a small amount of wiggle room. The training does not expire after exactly 365 days. You are compliant until the end of the calendar month one year after your last Annual Refresher training.
