3. Next, enter the coach's name in the designated field. This may be required for communication purposes.
4. Indicate the level you are registering for by circling one of the options: Elementary, K-8, Middle, High, or Special Ed.
5. For athletes, provide their names in alphabetical order, starting with the last name followed by the first name.
6. Select the appropriate date choice for the tournament from the tournament information sheet. This choice may be affected by the athlete's availability.
7. Identify the skills level by checking either Level 1 (less skilled) or Level 2 (more skilled) as applicable.
8. If applicable, check the box for special categories such as social or wheelchair to ensure the athlete's needs are properly registered.
9. Indicate if daily nursing services are required by checking the corresponding box.
10. For Skills Level 1 athletes only, check the appropriate box for the 8' basket and the 2-handed dribble if these apply.
11. Lastly, do not forget to enter the BSAT score, ensuring that this field is not left blank. Refer to the tournament information sheet for detailed scoring instructions.
12. After completing all sections, review the form for accuracy. Save any changes made, and you may also download, print, or share the completed form as needed.
