What is this form? To help the government fight financial crime, Federal regulation requires certain financial institutions to obtain, verify, and record information about the beneficial owners of legal entity customers.

The signatory is the individual opening the account. If the individual across the desk and providing the documentation is a beneficial owner, she would sign it. If the individual is nothing more than the company's "runner," he would sign it.

A beneficial owner is defined as any individual who owns either directly or indirectly 25 percent or more equity interest in a legal entity.

Business name. Business address or primary residence address. Date of birth. Social Security Number (as applicable) The name of the issuing state or country. Passport or driver's license number for the Beneficial Owners and Control Person as applicable.
