Follow the steps to successfully complete the application form.

1. Press the 'Get Form' button to access the Midwest Chapter Membership MIA Application online and open it for editing.
2. In the Company Information section, fill in the company name and complete the mailing address fields, including city, state/province, and ZIP/postal code.
3. Provide the work phone number and fax number in the designated fields, along with the company website.
4. Identify the primary chapter contact by entering their printed name, title or position, and email.
5. List names and emails of additional company representatives who should receive notifications about upcoming chapter events, including their titles/positions.
6. If applicable, provide your MIA membership number. Note that companies may join the chapter without being an MIA member for the first year.
7. Choose the membership category that applies to your company by checking all relevant boxes, such as Stone Producer or Installer.
8. Select the appropriate membership level and record the total amount due. Remember that dues are collected annually.
9. Indicate the method of payment for the first year's dues, and if using a credit card, fill in the card details, including the name on the card and verification code.
10. If the billing address for the credit card differs from the company address, provide the necessary information.
11. Obtain the primary chapter contact’s signature and the date, confirming that they will be the primary contact for all MIA chapter business.
12. Review the completed form to ensure all information is accurate, then proceed to save changes, download, print, or share the application as required.
