Fetal Death Reporting Requirements What is mandatory to report? • Weight of the fetus is 350 grams or more. Oregon collects reports and compiles statistics for each fetal death of 350 grams or more or, if weight is unknown, 20 weeks' gestation or more. Please view our current Fees. An Illinois-licensed Funeral Director must be contacted in order to complete a "Fetal Death. Certificate" within 7 days of the death. Fetal death certificates are not open to the general public. You must be eligible to receive a certified copy of a fetal death certificate.
