7. Ensure all supporting documentation, including National Board Scores and license verifications from other jurisdictions, are included. These must be submitted directly by the respective agencies.
8. If applicable, provide evidence of clinical examination passage within the last five years from recognized testing agencies.
9. Upon completing, review your application thoroughly for any missing information. Then, submit the application and fees to the designated address for the Oregon Board of Dentistry.
