Follow the steps to complete your online evaluation form

1. Click the ‘Get Form’ button to obtain the form and open it in an appropriate editor.
2. Begin by entering your name and Employee ID number in the designated fields. Make sure to fill out the date accurately.
3. Select the type of evaluation you are undergoing by checking the appropriate box: Permanent Status Evaluation, Scheduled Evaluation, Off-cycle Evaluation, PAR Follow-Up, or Interim Evaluation.
4. Indicate your department or school and your position in the respective fields.
5. In the Part I—Core Competencies section, review each competency and provide examples/evidence of how you or the employee meet or do not meet each competency. Use the CEIJ format if preferred.
6. For each competency, select the appropriate rating by checking either 'Meets Competency' or 'Does Not Meet Competency'.
7. In Parts II and III, add any additional comments and suggestions for professional development that support the evaluation.
8. Finally, in Part IV, indicate the final rating by checking the appropriate box and ensure comments supporting this rating are included.
9. The evaluator must sign the form in Part V, and the reviewer should also sign and date it.
10. After completing the form, save your changes, and you may choose to download, print, or share the document as necessary.
