7. If you are applying for a replacement certificate, fill out Section 6; otherwise, proceed to Section 7. In Section 7, ensure you have all items listed, including the necessary forms and documentation, before submitting.
8. Once all sections are completed, save your changes and choose to download, print or share the form as needed.
