12. Indicate the ‘Primary Position’ and ‘Secondary Position’ of the participant as relevant.
13. Review the ‘Informed Consent, Release, Waiver, Assumption of Risk and Hold Harmless Agreement’ thoroughly, ensuring understanding of its content.
14. Sign and date the ‘Signature of Parent or Guardian’ and ‘Signature of Participant’ sections.
15. Complete the ‘Camps & Clinics Authorization for Release of Participant’ section by filling in authorized individuals for pick-up.
16. Fill out any allergies or health concerns in the provided section for the safety of the participant.
