6. Answer whether the acreage is 'Yes' or 'No.' If 'No,' you will need to specify the lot number or subdivision name.
7. Fill in the service city, township or village, and the county along with the zip code.
8. Provide the major cross streets near the service location for clarity.
9. If known, enter the name of the DTE Energy service planner and their contact information. Choose the corresponding planner region.
10. For each site address, fill in the house number, street name, lot/apartment number, service type, construction meter type, and voltage class as needed.
11. If you have additional site addresses, duplicate the section as necessary and repeat the required information.
12. Once all fields are completed, review the form for accuracy, then save your changes, and prepare to submit it.
