10. Record the date of execution of the contract and the age of the apprentice at that time.
11. Specify the name of the trade for which the apprenticeship is intended.
12. Indicate the duration of apprenticeship training in years and months.
13. Provide stipend rates for each year of training. Ensure to include the correct amounts for the first, second, and subsequent years of training.
14. If the apprentice is a minor, enter the guardian's name and relationship with the apprentice.
15. Fill in the name and address of the surety, followed by necessary signatures from the employer, apprentice, guardian, and surety.
16. Record the contract registration number as provided by the office of the apprentice adviser.
17. Once all fields are completed, review the document for accuracy. Save the changes, and you can choose to download, print, or share the completed form.
