The Carroll-Davidson GAD scale (CD-GAD) is a 12-item screening tool for Generalized Anxiety Disorder. The CD-GAD has a yes/no format and measures GAD symptoms occurring over the past six months. Carroll, B., & Davidson, J.R.T. (2000). Developed by Jonathon Davidson, MD at Duke University Medical Center.

Score 0-4: Minimal Anxiety. Score 5-9: Mild Anxiety. Score 10-14: Moderate Anxiety. Score greater than 15: Severe Anxiety.

The metacognitive model of GAD (Wells, 1995, 1997, 2009) proposes that both positive and negative metacognitive beliefs need to be modified to enable people to disengage from worrying in response to trigger thoughts.

At the heart of the cognitive avoidance model of generalized anxiety disorder (GAD) are the notions that individuals with GAD avoid mental imagery and the emotions associated with the imagery; and that worry is a behavior that facilitates this avoidance.

The seven items assess (1) feeling nervous, anxious, or on edge; (2) being able to stop or control worrying; (3) worrying too much about different things; (4) trouble relaxing; (5) being restless; (6) becoming easily annoyed or irritable; and (7) feeling afraid as if something awful might happen.

GAD-7 total score for the seven items ranges from 0 to 21. 0–4: minimal anxiety. 5–9: mild anxiety. 10–14: moderate anxiety. 15–21: severe anxiety.

The Generalized Anxiety Disorder scale (GAD-7) is one of the most frequently used diagnostic self-report scales for screening, diagnosis and severity assessment of anxiety disorder.
