- Fill in the Departure, Destination, and Alternate fields with 2-11 alphanumeric identifiers, which can include Airports, NAVAIDS, or coordinates.
- Document your Route of Flight using published routes, latitude/longitude points, and separate consecutive fixes with the characters 'DCT'.
- Provide details in the Other Information field for any additional necessary clarification, including remarks prefaced with RMK/.
- Input optional information about Survival Equipment, Jackets, and Dinghies if applicable, while noting that their inclusion is highly recommended.
- Complete the Elapsed time and Total Time fields as required, with four-digit entries for each.
