- In the data owner (sponsor) or data designee section, only the data owner may request a new PHI shared folder or add users to an existing folder. Fill in their name, title, department, email address, and eRaider ID.
- If requesting a new PHI shared folder, choose ‘YES’ and enter the existing or desired shared folder name.
- To signify approval, ensure to return the completed request form along with an email indicating an ‘approved’ request to the provided email address.
- Once the form is completed, save your changes. You can download, print, or share the form as necessary.
