- In Block 18, select the College or University you are attending. Enter the Total Credit Hours Enrolled in Block 19 and specify the Term for your enrollment in Block 20.
- Finally, indicate the Year of your attendance in Block 21. Review all your inputs for accuracy before proceeding.
- After completing Part A, proceed to Part B where you will confirm your understanding of the program's requirements and provide your signature and date.
- In Part C, your Battalion Career Counselor will certify and sign your application. Part D requires the Education Representative’s signature and date for verification.
- Part E is for use by the Cashier's Office at the college or university. After all fields are completed, make sure to save any changes made to the form. You can download, print, or share the form as needed.
