5. Sign or provide your thumbprint in Part II to affirm your authorization.
6. Part III must be completed by the bank. This section requires bank verification to certify the accuracy of the provided account details and signature.
7. Review the completed form for any errors or omissions. Incomplete forms will be returned, so double-check all information.
8. Submit your signed form by post to the specified address: Tanjong Pagar Post Office, P.O. Box 889, Singapore 910824. Do not fax the form.
