The California Immunization Registry (CAIR) is a secure, confidential, statewide computerized immunization information system for California residents.

The Yellow Card shows proof of the shots needed to start school, day care, or a new job. If you move or change doctors, your new doctor can look up your/your child's vaccination record in CAIR. This helps your doctor know which shots you/your child had and which ones are still needed.

If you lose your vaccination record (Yellow Card), your doctor can quickly print a new one for you using CAIR. You can also contact the CAIR Help Desk at cairhelpdesk@cdph.ca.gov to have a new Yellow Card printed for you. The Yellow Card shows proof of the shots needed to start school, day care, or a new job.

Since the yellow fever card never expires, you can continue using the same one for life. What happens, though, if you lose or misplace your card? There's no need to panic if this happens. Try to get in touch with the healthcare facility where you were vaccinated.

In California, the blue California School Immunization Record card is a tool pre-kindergarten facilities and schools can use to record information required to be on file for every child enrolled. This record card is sometimes referred to by its initials (CSIR), number (CDPH 286), or color (blue).

For privacy and protection of your personal health information we only accept records uploaded in your patient portal. You will need to log in the patient portal and upload your immunization records in the medical clearance section.

The date the VIS is provided (i.e., the date of the visit when the vaccine is administered). The office address and name and title of the person who administers the vaccine.
