8. Input the player's birth date, weight, and height, as well as their batting orientation (right or left) and preferred fielding position (right or left).
9. Insert the name of the school the player attends.
10. Provide parental information, including the names, addresses, phone numbers, and email addresses for both parents or guardians.
11. Indicate whether you, as the parent or guardian, are willing to volunteer with the league by checking 'Yes' or 'No' under the various volunteer roles available.
12. Review all entries for accuracy before finalizing the form.
13. Upon completion, save changes, and choose to download, print, or share the form as needed.
